Member forms

If you need to inform us of an update, you can now use the Pensions Portal  to review and update information such as:

  • Your name, title and marital status

  • Your postal address, email address and contact telephone number

  • Your nominated beneficiaries

  • Your bank details (if you are in receipt of your pension)

Our other forms are available to print and download below, however if you require assistance or a document in an alternative format (braille/large print, for example), please contact us.

If you wish to complete a form to grant a third-party individual or organisation authority to act on your behalf, please visit our 'Third-Party Authority' page.
